1. Why do you want to be a beta tester?
I want to be able to help devolp and make newer games better.

2. How old are you?
17 years of age. Oct. 13, 1992.

3. Do you know what the job of a beta tester is?
I understand that this job requires you to play the game and help test out buggs and find sertian ways to fix them.

4. Do you understand that any fooling around during beta testing will not be tolerated?
I understand.

5. Do you understand that any breaking of rules will result in you being removed from your group and possibly banned?
I submit myself to this user group and know any rule breaking may result in removal and ban.
